Swansea's Health Social Care and Well Being Strategy 2008-2011

We are now in our second year of the current HSC&WB Strategy. It is working towards the vision that Swansea in 2020 will be in a place where:-

  • Everyone is supported to achieve the best level of health and well being possible, living longer healthier lives
  • Appropriate care is delivered in the best location at the right time by a range of organisations working together
  • Communities and individuals are well informed and take responsibility for their own health and well-being

The current HSC&WB Strategy for 2008-11 has been devised based on the Health Needs Assessment (2007), which has four high level priority areas including:

  • redesign social care services for Older people
  • develop social care services for children
  • reshape mental health services
  • redesign acute (hospital) and community based care in Swansea

Further priority areas have been agreed, to prevent ill health, improve health and social care services for a range of population groups, and develop our workforce, IM&T and the way in which we secure and deliver services more jointly.

The Health Challenge Swansea partnership is supported by a number of partnership arrangements, who have worked with a wide range of people in local organisations to formulate the second strategy.
